By participating in MIYU BEAUTY LLC’s yoga and pilates classes, health programs, workshops and/or other wellness, body work, therapy, exercise and healing arts activities (collectively, the “Activities”) you acknowledge and agree to the following terms and conditions:

Health and Safety:
a. I understand that the Activities involve physical activity and may require physical exertion. I certify that I am in good health and have no medical conditions or physical limitations that would prevent me from participating safely.
b. I acknowledge that it is my responsibility to inform the instructor of any injuries, medical conditions, or concerns that may affect my ability to participate fully in the class.
c. I agree to follow the instructions of the instructor regarding proper techniques, safety guidelines, and modifications when necessary.

Assumption of Risk:
a. I am aware that participating in the Activities carries some inherent risks of injury or property damage.
b. I voluntarily assume all risks associated with my participation, including but not limited to physical injury, accidents, or illnesses that may arise from my participation in the classes.
c. I understand that the instructor and the facility will take reasonable measures to ensure a safe environment, but they cannot eliminate all risks.

Release of Liability:
a. I release, discharge, and hold harmless the instructor, the facility, and their respective employees, representatives, and affiliates from any and all claims, liabilities, damages, actions, or causes of action arising out of or in connection with my participation in the Activities
b. I, my heirs, or legal representative of such forever release, waive, discharge and covenant not to sue MIYU BEAUTY LLC or any of MIYU BEAUTY LLC’s instructors, or the leaseholder or owner of the building for any injury or death caused by their negligence of other acts.
c. This release includes, but is not limited to, any claims for personal injury, property damage, or loss arising from negligence, breach of contract, or any other cause of action.

Personal Belongings:
a. I understand that I am responsible for my personal belongings brought to the facility. I release the instructor and the facility from any liability for loss, theft, or damage to my personal belongings.

Photographic and Video Release:
a. I grant permission to the instructor and the facility to take photographs or videos during the classes. I understand that these photographs or videos may be used for promotional purposes, and I release all rights to compensation or ownership.

I have read this liability waiver and fully understand its terms and conditions. I voluntarily agree to be bound by its contents and acknowledge that I am waiving certain legal rights by signing it. I affirm that I am 18 years of age or older, or that my parent or legal guardian has read and signed this waiver on my behalf.
